18-21 Transition Program Follow-up Procedure

Students new to the district who had an IEP in high-school may be best served in the Transition Program. Enrollment begins (and is completed) at the District 205 Center Student Services Department. Afterward, an IEP meeting will be scheduled to review records and determine in placement in the program is appropriate.

Once the required enrollment forms have been submitted and the online registration process is completed at the school, parents may contact the Special Education Supervisor at York High School to obtain additional information specific to the student. This may include information about free or reduced lunch, bus route and schedule, additional questions parents may have, or arranging a brief tour of the school.
